Hours of Operation: One Shoreline in Home Child Care is open from
7:30 a.m. until 5:30 p.m. Monday through Friday. If care is needed beyond these hours please
discuss possible accommodations for before/after hour or occasional weekend
care.
Hours of
care will be contracted from child to child. No childcare will be provided on
Weekends or on the following holidays:
New Year’s Day, Memorial Day, Independence Day,
Labor Day, Thanksgiving and the Day After, Christmas, Christmas Eve.
The
above are paid holidays for The Provider if they fall on a contracted day for
your child. If they fall on a weekend they will be observed the Monday after if
Holiday falls on a Sunday or Friday before if holiday falls on Saturday.
In
addition, The Provider will close for 2 weeks of vacation per year and 3 paid
personal/sick days per year. Notification of at least 60 days will be given
prior to any closed days, with the exception of emergencies or illness. Please
have a backup childcare provider for these occasions. The Provider is not
responsible for finding alternate care for your child, although she will try to
have a backup. In the event that I will be away for a period of time less than
three hours (usually for Doctor appointments only) substitute care will be
provided by a backup provider that I will arrange.
After
reading each policy please initial and date that you UNDERSTAND what the policy

Rates:
Weekly / Part Time Weekly / Monthly
Part time care is less than five hours per day, five hours and
over is considered full time.
Infants
birth to 18 months of age - $170 per week / $100 per week / $600 per month
Toddler
19 months to 36 months - $160 per week / $90 per week / $600 per month
Preschool
37 months to 6 years old - $135 per week / $80 per week / $550 per month
School
Age 7 years to 12 years - $120 per week / $70 per week / $550 per month during the school year,
children attending school full-time (8am - 3pm) will be $350 per month
Part time hourly rates: $4.75 an hour for under 18 mos, $4.50 an hour
for over 18 months but not yet potty trained, and $3.75 an hour for potty trained
children.
Fees:
All Children will be assessed a $35 non-refundable enrollment
fee.
Late
Fee: $2.50 per 10 minutes per child after contracted pick up time
Late
Payment Fee: $10.00 per day that payment is not received
Returned
Check Fee: $30.00 plus any additional costs I incur for a returned check
Payments
*All
payments are due on the 1st of the month by 5:30 PM after the 5th day of the month 5:30 PM, the
late fee will be assessed. If payment is not made within 3 days at drop off,
your child will not be accepted into care until payment, including all late
fees, is made. If a period of 7 days passes without payment received, the
contract will be terminated, the position filled, and the collection process
begun. You will be responsible for any costs related to collection of the
childcare fees.
*Payments
must be made in cash or check form and a receipt will be given. Paypal and
credit card payments are also accepted with a 3.5% additional service fee. A
year end statement of all childcare fees paid will be provided within the first
4 weeks of the New Year, to the address that is on the contract. A fee of
$30.00, plus any additional costs I incur, will be charged to you for a
returned check. All future payments will then be made by cash, or official bank
check.
*Childcare
fees are due regardless of whether or not your child attends. You are paying
for a position, as well as a service. If you are hourly, the normal hours you
are contracted for are paid weekly in advance every week regardless of attendance
(unless vacation days are being used, with 60 days notice having been given).
These fees are due regardless of unforeseen or arranged absences such as
illness, monthly club meetings or appointments. If a part time hourly child is
not in attendance, the normal tuition is still due. I will accept DHS
assistance payments, after the
*All
childcare services will be contracted. The contract is a legal document
obligating me to provide a service for you and obligating you to pay me for
that service. There are other requirements in the contract. I urge you to
thoroughly read the contract and realize that it is legal and you will be held
liable for each item of the contract. By signing it, you are accepting it in
all its terms.
*A
position at The Home Day Care will be considered open until the payment for the
first weeks’ fees are received. This non-refundable deposit is half your
tuition for your first week and half a non-refundable holding fee.
*You are
allowed up to 2 weeks of vacation at 50% tuition. Any tuition taken beyond 2
weeks in a calendar year is 100% tuition. If you have school age children
please consider this when making plans of when to take your vacation time.
Provider will take 2 weeks in a calendar year which is also to be paid at 50%
tuition.
Daily Doings
Drop Off/Pick Up:
1.
Please do not leave your car running. Please do not allow your child to play
near the vehicles at any time.
2. Your
child must exit my home with you.
3. I
assume responsibility for your child only while he/she is in my care during
regular hours.
4. No
child will be allowed to leave with anyone except the parent, unless indicated
on the alternate pickup list, without written permission from the parent.
Anyone unfamiliar to me will be required to show proof of Identification.
Please make the alternate pick up person aware of the requirements.
5. It is
normal for your child to cry on arrival, especially for the first few weeks.
Please make your goodbye brief and tell your child exactly when you will be
returning. The crying usually stops within seconds of your departure. You are
welcome to listen outside the door. Please never leave without telling your
child goodbye and feel free to call me and check on them at a later time.
6.
Please be in control of your child during drop off and pick up times. This is a
time of testing when two different authority figures are present (parent and
provider) and this situation will be tested at one time or another to see if
the rules still apply. I will remind your child if inappropriate behaviors are
being displayed. Children of all ages adjust to transitions from one activity
to another differently. Most do not like to be too rushed and most do not like
to wait too long once they are ready to depart. If you need the children to be
ready to leave, please call ahead and we will clean up toys in advance of you
coming.
7. If
the person picking up the child appears to be under the influence of alcohol or
drugs, another authorized person will be called to pick-up both the child and
the adult.
8. Court
Order: If there is a court order keeping one parent or guardian away from the
child, I must have a notarized copy of that court order in my file to that
effect. Otherwise, I cannot prevent the non- custodial parent from picking up
the child.
Late Pickup:
If the
child is picked up more than 8 minutes late, from the contracted or arranged
time, the Late Fee will be assessed. No Child will be accepted into care prior
to their contracted drop off time.
Late
Fee: $2.50 per 8 minutes per child.
If a
parent is 30 minutes late and I have not received a phone call to tell me when
the expected pick up time is, I will call the parent numbers that I have. If I
am unable to reach anyone, I will try again at 45 minutes late. If there is
still no answer or the parent has not shown up at this time the child’s
alternate pick up person will be called to come get the children. Continuing
efforts will be made to contact someone. If after 3 hours no parent or
alternate pickup person is able to be found, the children will be made
comfortable to sleep overnight and efforts will continue until a reasonable
hour. If the parents have still not shown up by the next morning, the police
will be notified. Please make sure your emergency contact information is
correct and updated as needed. If this situation ever occurs I will make every
effort to care for your child as normal and help them to not become worried. At
no time will children be blamed for their parent’s lateness.
Open Door:
You are
invited and welcome to visit anytime your children are present. You are asked
to avoid visiting during Nap/quiet time (about noon - 3 pm) as much as
possible. Parents are also free to call The Provider at any time. If I do not
answer, please leave a message, and I will call you back as soon as we are
finished with the current activity.
Sick/Vacation Days:
In the
event that your child is ill and needs to miss a day, please call or text 1
hour before arrival time (but no calls earlier than 6:00 am.) If your child has
an extended illness, a discount may be arranged to hold the position at my
discretion.
If you
are taking a maternity leave and want to secure your spot for both the new baby
and keep your spot for the current child, arrangements must be made before the
birth of the new child. A non-refundable deposit will be required to hold the
new baby’s spot as with any newly enrolled child. This deposit will be one
week’s tuition, half to be applied to the 1st week’s tuition, and
half a holding fee. Please remember when arranging for the older child’s leave
time, that this is my source of income and I can not take a complete pay cut
for the entire leave time, so a portion of your tuition will be due regardless
of attendance.
If your
child will not be attending due to vacation, you will be charged 50% of the
normal weekly rate to hold your child’s position. You are allowed 2 weeks of
vacation at this discount, any vacation taken beyond these 2 weeks in a
calendar year will be paid at normal tuition. I require 60 days notice (the
same in which I give you) of vacation time and payment for that time in
advance.
The
Provider will also take 2 weeks of vacation to be paid at 50% pay. In the event
that the Provider is sick (or her child) and unable to provide services, the
provider will use one of her 3 paid sick/personal days. If those days have been
used then a portion of your tuition for that week will be refunded to you.
Please make sure you have a back up care provider ready for these occasions. I
will do everything in my power to avoid such an occurrence, but am not able to
guarantee perfect health.
Discipline:
No child
will be hit, spanked, belittled, or otherwise intimidated at The Provider’s –
even with parental permission. No corporal punishment will be used. Children
will be treated with courtesy, respect, and patience. Discipline will be
according to age and understanding level. Younger children, babies and
toddlers, will be redirected to another activity. Older children will be given
time outs depending on the severity of the offense. If a child becomes a
persistent behavior problem, I will address it with you and we will try to
resolve it together.
AT NO
TIME WILL A CHILD BE SUBJECTED TO PHYSICAL PUNISHMENT OR SHAMING, FRIGHTENING
OR HUMILIATING METHODS, OR ANY TYPE OF VERBAL ABUSE, THREATS, DEROGATORY
REMARKS, OR DEPRIVATION OF A MEAL OR ANY PART OF A MEAL BE USED. NO CHILD WILL
EVER BE PUNISHED FOR TOILET ACCIDENTS.
Meals:
I
provide nutritionally balanced meals and snacks for your child. In an effort to
serve the best food I try to buy whole grain, whole fat and fresh organic foods
in a wide variety of meal options. Your child’s nutrition is an important part
of their life; I aim to give them a healthy start. Please do not send any food
or drink with your child without prior discussion with the provider. If your
child arrives after a meal or snack has been served, he/she will wait until the
next meal/snack time to eat. Children are encouraged to use this time to share
their experiences with each other. Manners are taught, and practiced during
this time as well. Please list on the child information sheet any food
allergies the child may have. If your child needs a special diet, the parent
must furnish these foods. All formula/breast milk is to be provided by the
parent. Infants will be started on solid food by 6 months old and on table food
by 12 months of age. I plan participate in the government food program for
child care providers. This reimburses me for a small portion of the food cost
each meal but does not cover anywhere near what is spent.
I
provide Breakfast, lunch and two snacks to the children while in my care. Two
examples of snacks and meals they will receive are as follows:

Activities:
Age
appropriate activities are scheduled with flexibility allowed to respond to the
needs of the individual child and day. I will offer times for outside play,
crafts, stories, instruction, and naps appropriate to the child's ages,
interests, and abilities. I will provide your child with tender loving care,
understanding, patience and guidance in a happy family setting. Chores, such as
picking up, putting dishes in the dishwasher, will be a normal part of the day.
I provide a preschool curriculum, developing large and small motor skills. We
do many arts and crafts where the focus is on the process, not the product.
Free play is an important part of a child's early years. It is here that they
learn social skills that will be needed the rest of their lives.
Circle/Learning Time:
Time to
read a short book then learn or practice skills. Children will be have all
skills necessary for kindergarten introduced during circle time in a gradual,
fun manner. The amount of time spent on skills varies by age, interest and
abilities. Some days we will cover a lot, other days nothing at all, it all
depends on the children. By the summer before kindergarten all necessary skills
will have been introduced.
Outside, Beach Play:
We will
try to get outside every day regardless of the weather is good so make sure to
dress your child appropriately and send outside items in the winter. I have a sprinkler and small pool for summer
water play and our close proximity to the beach is a large part of our
program. I will provide shade for
children while playing at the beach and children will be allowed in ocean water
in safe conditions up to their knees after showing they can follow beach
etiquette and safety rules. A high SPF
Sunscreen will be applied liberally to any open sun and children will always have
water available and encouraged to drink on hot days. A swimsuit/swim diapers
should be sent with child from May-September.
Outdoor Exploration &
Activities:
Growing
up in the Pacific Northwest playing, exploring and having free time in nature
is a large focus of our daily activities, outdoor play is to be expected on
most days. We will go for frequent
hikes, beach walks, bike rides and more.
Occasionally opportunities for summer camps or swim lessons might be
optional activities that your child is welcome to participate in at your own
cost. I will provide as much notice as
possible.
Nap/Rest Period:
All
children are REQUIRED by the Provider to have a nap/quiet period. No child is
forced to sleep, however they must remain quiet. Older children, and those who
wake early, will be allowed to read a book, play quiet card/board game until
Rest Time is over. Please try not to schedule pick-ups or visits during this
time to lesson disturbance to the resting children. No child will be woken up
from a nap unless the parent has arrived to pick up the child.
Toys:
Please
do NOT send any toys from home (especially toys with screens) with your child
unless special arrangements have been made. If your child needs a special toy
or item for sleeping, it will be allowed, but it will remain put away until Nap/Rest
Time. The Provider assumes NO responsibility for lost, stolen, or broken toys
from home. Should the child deliberately destroy my toys or other property
through misuse or willfulness, the parent will be required to replace it within
two weeks, or pay the cost of the toy.
Dress Code:
Please
dress your child appropriately. The activities may be messy. Do not send your
child in clothing that you do not want stained. I use washable art supplies,
but caution never hurts. Weather permitting; we will spend a lot of time
outdoors. You also need to supply a complete change of clothing in case of an
accident; I do not supply clothing for day care children. If the child has no
spare clothing, the parent will be called to bring some. If there is a special
occasion that calls for special clothing, (a visit or party right after child
care or a trip to the photographer) please send the special clothing with your
child and I will help them clean up and get dressed prior to your picking them
up at the end of the day.
Toilet Learning:
I will
assist you in toilet training your child with the understanding that it will be
successful only if we work together. Send your child ONLY in easy on/easy off
clothing until they are able to completely undress and dress themselves. I
require at least 3 complete changes of clothing during Toilet Learning. I do
not launder soiled items and will send them home in a plastic bag. Please
replace any clothing sent home the next day. Underwear is not to be worn to
daycare until the child has been dry for at least one week and is initiating
toilet use.
Transportation:
Often,
especially during the summer we may plan a field trip or it may be necessary
for me to transport your child by car. All traffic and safety laws will be
followed. No child will ever be left unattended in a vehicle. The only time we
may leave without notice is in the case of an emergency, in which case a note
will be placed on the door leaving instructions where the children can be
found.
School drop off times are 7:45-8am and 2:45pm-3:15pm we
will provide transportation to Crestview Elementary and Yachat's Preschool
during the school year. Unless it is an emergency, you will always be notified
prior to any field trip from The Provider. You as parents reserve the right to
refuse. If I do not have your permission to transport your child, you are
expected to pick up your child immediately and make alternate childcare
arrangements for that day. Full tuition would still be required in such a
situation.
Pets:
The Provider
has 1 dog (Stella, German Wirehaired Pointer) and 1 beta fish (Aurora). They
are kid-friendly, lovable creatures. They have been certified as healthy by a
veterinarian and are current on all immunizations. Stella is still a very
excitable meeting new people and has a tendency to jump on new arrivals, but
quickly calms down and spends most of her day lounging around. She will play
with the children when we are not doing sit down learning activities or when we
are outside. They are very well loving animal and loves children and the
attention they receive from them.
Hitting or any abusive behavior of animals is strictly forbidden and
taken very serious if child’s behavior is not remedied.
Smoking:
The
provider does not smoke, smoking is NEVER permitted inside the provider’s house
or within 10 feet of the door or any open windows.
Fire Arms:
Firearms
are not present in this facility.
Poisons:
There
are no poisonous plants at this facility and all household cleaners are
securely placed out of reach from children.
Home Safety:
This
home is a 1940’s beach cottage and there is lead paint on the property, to my
knowledge it has been fully painted over and I will be maintained as such. Drinking and cooking water is also filtered
before use. The deck is slated to be
repaired in the fall 2014, and until then children will not be allowed to play
on the deck. Stairs are steep as well
and children will not be allowed to use the stairs down to the basement. If activities are done in the lower level of
the home/basement we will use the downstairs door to access it.
Holidays/Birthdays:
We honor
major holidays and all children’s birthdays. If you would like to bring a
special treat for the children, please arrange this with the provider. You are
more than welcome to participate in ANY activities we have planned. Please
inform the provider if you do not wish for a holiday (Christmas, Halloween,
etc.) to be focused upon.
Television:
I do not
allow TV viewing except during specified times of the day when I let the
children pick a child appropriate video or pbs program to watch during
inclement weather. Children are NEVER required to sit and watch TV, and TV is
not offered in place of Free Play or Learning Activities.
House Cleaning:
My house
is not always spotless. My main concern is the care of the children. I clean my
house during Nap/Rest Time, if all are asleep, during non-business hours, and
occasionally with children's help. The children will help pick up toys before
meals and naps. Toys are sanitized and cleaned as needed.
Health & Safety
Emergencies:
Fire:
There
are fire extinguishers located in my kitchen and downstairs in the basement. If
there is a fire that cannot be extinguished quickly, the children will be
evacuated immediately before calling 911. We will practice fire drills monthly
so the children will be prepared in the event of a fire.
Power outage:
There
are flashlights located throughout my house. If the power remains out for some
time and the safety of the children is at risk you will be called to pick up
your child. We do have a fireplace and
lots of emergency gear should sheltering in place be required.
Natural Disasters:
Living
on the coast we will practice earthquake and tsunami drills, discuss earth and
water safety and have routes planned for evacuation to Crestview Heights
Elementary in case of natural disaster.
Changes to Policies:
Changes
may be made to these policies as needed with 2 weeks notice. The policies,
contracts, consents, and forms will be reviewed and updated, if needed, yearly
in January. Please give written notice of any changes that may occur,
especially of name or address, or of updated immunizations.
Confidentiality & Photo
Release:
The
information you supply to The Provider will be kept confidential. I will, at
all times, respect your privacy. Before any of the information is released to
outside persons, the parents will sign a release form. (This does not apply to
law enforcement) I also frequently post
photos on social media, with state agencies and child care resources programs
and grants and happy to share in this art with you, if you feel
comfortable. Please sign the media
release according to your preferences.
Enrollment Requirements:
Before
enrolling your child in The Provider’s Care there are several things you must
do:
*Read
through and become familiar with the Policies. You will be required to sign a
form that indicates you have read, understand, and agree to ALL the Policies as
outlined.
*A
complete immunization record must be on file. If the immunizations are not up
to standard your child will not be permitted in the Day Care. The immunization
record will be valid for two years, unless your child is still receiving
immunizations. Please be sure to give proof of updated immunizations as they
occur. A chickenpox vaccine is now required by DCFS for all children unless
they have had the disease.
*An
acquaintance visit must be made. I will not care for children unless they have
had a visit to The Provider’s Home to become familiar with me and the service
provided.
*All
appropriate forms must be filled out, signed, and on file PRIOR to admission.
All necessary forms/consents will be given to you in your admission package.
*One
week’s tuition is due as a non-refundable deposit. Half of this will be applied
to your first week’s tuition; the other half is considered a holding fee.
Termination of Care:
After a
TWO-WEEK TRIAL PERIOD, either party can only terminate care with 2 weeks
notice. I reserve the right to immediately end care for non-payment, failure to
respect me, my home, my neighborhood, behavior of the child which is harmful to
the physical or emotional well being of the other children, or failure to abide
by my policies. If you terminate care, you will be responsible for payment of
the final 2 weeks of care whether or not your child attends.
Child Abuse/Neglect:
I am
required by law to report any suspected signs of child abuse and/or neglect.
Supplies:
You will
need to provide the following things

*Diapers,
at least 5 per day (if applicable)
*1 box
of baby wipes, at least 80 - 100 count. Please bring a refill box the first
time and then refill packs as needed (if applicable)
*2
pacifiers, if needed
*Formula
or breast milk, if applicable, and bottles. Please label with your child’s
name. Bottles are rinsed and sent home, not washed here.
*2
changes of clothing (4 if we are toilet training) to be left here. This
includes shirt, pants, and underwear/onsie and socks. Please replenish if these
are sent home dirty, so that at least one set remains at all times.
Note:
All of
the above items are required for me to care for your child, so they will need
to be brought daily or left here. If you choose not to supply me with the
required items, I will purchase them for you and you will be responsible for
the cost and my time of gathering the supplies. This would be considered a
premium service and be charged as such.
Illness:
Please
ask for specifics on my illness policy. Children will need to be kept out of
daycare for vomiting (24 hours throw-up and fever free before returning
please), uncontained diarrhea, or any illness I deem them needing to be kept
home including communicable diseases including, head lice, undiagnosed rash,
green phlegm/boogers. A doctor’s note may be requested in certain circumstances
in order for a child to return to care.
Medications:
No child
will be given any medication, prescription or over the counter, unless written
permission is given by the parent and the Doctor. Prescription medication shall
have the child’s name, name of medication, Doctor’s name, name of pharmacy,
prescription number, date, and directions for administering. The medication
must be in the original container as dispensed by the pharmacy.
Medical Emergencies:
Although
supervision is constantly given, I cannot be by the child’s side at all times
to prevent falls, tripping, bumps, blows from other children, etc. I am
certified in infant, child and adult first-aid and CPR and was also a former
Red Cross Instructor. If the child is
injured in a non-life-threatening way, I will assess the child and provide home
first aid (antiseptic, bandages, arnica cream, antibiotic cream, Benadryl
topical cream etc..). If the injury is more serious, the parent will be
notified so the child can be transported to the hospital or doctor’s office.
(I.e. needs stitches, broken arm, or dislocation, etc.) All costs associated
with injuries to the child will be the responsibility of the parent. In case of
a medical emergency, I will attempt to contact you immediately. If I am unable
to reach you, I will start calling the people designated as your emergency
contacts. If I am unable to reach you or your emergency contacts, I will
transport your child to his/her doctor or to the hospital, if necessary. If
immediate intervention is required, and will take appropriate action including
calling 911 and having your child transported to the nearest Hospital. You or
your family insurance is/are responsible for the cost of medical help or
treatment due to accidents or illness while in child care.
